- Ration Card with address *
- CGHS/ECHS Card
- Water Bill (not older than last three months)
- Telephone Bill of Fixed line (not older than last three months)
- Electricity Bill (not older than last three months)
- Registered Sale/Lease Agreement
- Freedom Fighter Card having photo
- Arms License
Photo Identity Proof
(Any One of the Following):
- Aadhaar Card/e-Aadhaar letter downloaded from UIDAI website
- Driving License

- Ration Card with Photo, for the person whose photo is affixed *
